Volunteers Needed for the 2014 Community Health Assessment (CHA) Process

Forsyth County Department of Public Health (FCDPH) in collaboration with Forsyth County Healthy Community Coalition, Novant Health Forsyth Medical Center and Wake Forest Baptist Health will be conducting a community wide assessment to identify priority health issues and plan interventions to improve the health of the community. This is required by the NC Health Department Accreditation Board and the Affordable Care Act/IRS 990 Schedule H Requirements for Not for Profit Hospitals to conduct an assessment every three years.

We are looking for community members to represent:

  • Faith based organizations
  • Teachers and/or school personnel
  • PTA
  • Businesses
  • Neighborhood associations
  • Teenager
  • Seniors
  • Media
  • Civic and social organizations
  • Voluntary agencies
  • Local universities and colleges
  • Different ethnic and racial groups

Our goal is for the team to be a broad representation of county residents and representatives from strategic agencies and organizations that have a good understanding of the county and are willing to commit the time and effort necessary to make the process a success.

The first meeting is scheduled for Monday January 27, 2014 from 11:00am-1:30pm at the Forsyth County Department of Public Health located at 799 N. Highland Avenue, Winston-Salem, NC 27102.

The 2014 CHA Team will meet monthly till October 2014 at various locations.
